PROBLEM
A 16-year-old boy presents to your emergency department complaining of right hip pain. What is your diagnosis based on this frontal film of his pelvis?

ANSWER
This patient has congenital right hip dysplasia, best demonstrated by the lateral position of the femoral head in relationship to the acetabulum. Also, comparing the right acetabulum to the left reveals abnormal positioning and configuration. Congenital hip dysplasia is not an unusual condition, but it will lead to early degeneration if it is not corrected surgically.
